Travel Document
Filipino nationals who held lost, expired or mutilated/damaged passports, as well as newborn Filipino children (with no passport), who URGENTLY need to travel to the Philippines, may apply for a Travel Document. A Travel Document can be used for one-way direct travel to the Philippines only. Travel Documents cannot be used for travel to countries other than the Philippines.
The requirements are:
- Personal appearance of the applicant (and parents if applicant is a minor)
- Fully accomplished Travel Document application form (may be downloaded from Embassy website or obtained in the Embassy upon arrival)
- Three (3) passport-size photographs
- Affidavit of explanation for the application of Travel Document (may be downloaded from Embassy website or obtained in the Embassy upon arrival)
- Situation-specific requirement:
- For lost passport, submit
- Printed copy of the lost passport
- If passport copy is unavailable, submit a copy of the Philippine Statistics Authority (PSA) Birth Certificate; the PSA Marriage Certificate is also required if the married name was used in the passport that was lost.
- Police Report preferably with English translation. If the Police Report is written in Japanese, a translation fee of ¥3,750 will be collected in addition to the travel document fee.
- For expired passport: Present the original expired passport with a photocopy of the data page.
- For mutilated/damaged passport: Present the original passport with a photocopy of the data page AND a photocopy of the damaged/mutilated page.
- For newborns without a passport, submit:
- Original and photocopy of the child’s Report of Birth (ROB)
- Original and photocopy of the parents’ passports (if child is legitimate), or the mother’s passport (if child is illegitimate)
- For lost passport, submit
- Confirmed flight itinerary
- Original and photocopy of a valid government-issued ID (residence card, etc.)
- Proof of urgency of travel (if applicable to the applicant’s situation):
Death of immediate family member (copy of death certificate)
Medical reasons (copy of medical certificate)
Legal proceedings (copy of the subpoena or notices of hearing)
Deportation papers from Japan Immigration (copy) - Cash payment of the travel document: (click here for the updated schedule of fees)
a. Travel document fee (+ Affidavit fee): ¥8,250
b. Expedite fee: ¥1,500
The Travel Document is issued within 2-3 hours. Applicants are advised to apply for the travel document at least 24 hours before their flight. If the flight is scheduled during a a weekend or holiday, applicants should apply at the Embassy on the last working day before the weekend/holiday.
There is no need for an appointment when applying for Travel Document.
