  3. Personal Appearance
  4. Current ePassport with photocopy of data page
  5. Self-addressed 520 Letterpack
  6. Payment of JPY 7,800.00

NOTE: If the passport still has validity of at least 2 months on the day of the application, Applicant may submit a self-addressed Hagaki (Japanese Postcard) in order to request for the passport to be cancelled when the new passport arrives. If requested, the Hagaki will be sent to the client when the passport arrives. The applicant must send the passport to the Embassy through a 520 Letterpack. Once the passport is received, it will be cancelled, and the cancelled and new passport will be returned to the client by mail.

Additional Requirements Required for the Following Cases:

For Change of Name in the Philippine Passport or Discrepancy in Name (such as change of name due to marriage or reverting to maiden name)

    2. Original PSA authenticated documents that will support the change of name.
      1. Marriage Contract
      2. Annotated Birth Certificate
      3. Annotated Marriage Contract to show annulment/divorce/court ordered instruction
      4. Death Certificate of Spouse

NOTE: If woman opts to retain maiden name, a Marriage Contract is not required.
Additional requirements may be required.

For Holders of NON-ePassport Applications

  2. Any of the following Valid IDs with one (1) photocopy (click here for a list of valid IDs).

NOTE: If there is a discrepancy in the relevant documents presented then the Consular Officer may require additional documents such as an original PSA Birth Certificate.
