Report of Marriage (ROM)
A marriage contracted by a Filipino national in following prefectures in Japan must be reported in the Philippine Embassy in Tokyo within one (1) year from its occurrence: Akita, Aomori, Chiba, Fukushima, Gunma, Hokkaido, Ibaraki, Iwate, Kanagawa, Miyagi, Saitama, Tochigi, Tokyo and Yamagata.

Requirements
- Personal appearance of both parties
- Duly accomplished Report of Marriage (ROM) form (Click this link to download the form)
- Valid passport of both parties – original and four (4) copies of the data page
- Konin Todoke no kisai Jiko Shomeisho (Certified true copy of Marriage certificate) from the City hall – Original and four (4) copies (including ALL attachments, IF ANY)
Note: The Konin Todoke no Kisai Jiko Shomeisho may also be requested from the Legal Affairs Bureau, where the Japanese city hall is under, 2-3 months after its registration. - If spouse is Japanese – latest Family Registry (Koseki Tohon) showing the couple’s date and place of marriage – Original and four (4) copies
- If spouse is a foreign national, other than Japanese – Certificate of Acceptance of Marriage (Juri Shomeisho) – Original and four (4) copies
- Affidavit of delayed registration of marriage – for 1 year beyond date of Marriage
- Passport-sized photos – four (4) photos of husband; four (4) photos of wife.

ADDITIONAL requirements for those who did not apply for Legal Capacity to Contract for Marriage (LCCM) in the Embassy prior to marriage
For Single Filipino applicant:
- DFA Authenticated or DFA Apostilled PSA issued Birth Certificate – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued Certificate of No Marriage (CENOMAR) – Original and four (4) copies
(make sure that the details in the PSA CENOMAR are consistent or the same with the PSA Birth Certificate) – valid only with six (6) months from the date of issuance
For a Divorced Filipino applicant (An applicant whose previous marriage was divorced before contracting the marriage being reported):
- DFA Authenticated Judicial Decree of Absolute Divorce issued by a Philippine court with Certificate of Finality – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued Marriage Certificate or Report of Marriage with annotation – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued Advisory on Marriages (must reflect annotation on divorce decree) – Original and four (4) copies
(make sure that the details in the PSA Advisory on Marriages are consistent or the same with the PSA Birth Certificate) – valid only with six (6) months from the date of issuance - DFA Authenticated or DFA Apostilled PSA issued Birth Certificate – Original and four (4) copies
- If divorced to Japanese national – Koseki tohon of former Japanese spouse (Original and four (4) copies)
For a Filipino applicant whose previous marriage was Annulled before contracting the marriage being reported:
- DFA Authenticated Judicial Decree of Annulment issued by a Philippine court with Certificate of Finality – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued married Certificate/ Contract or Report of Marriage with annotation – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued Advisory on Marriages (must reflect annotation on annulment decree) – Original and four (4) copies
(make sure that the details in the PSA Advisory on Marriages are consistent or the same with the PSA Birth Certificate) - DFA Authenticated or DFA Apostilled PSA issued Birth Certificate – Original and four (4) copies(make sure that the details in the PSA Advisory on Marriages are consistent or the same with the PSA Birth Certificate) – valid only with six (6) months from the date of issuance
For widowed Filipino applicant:
- DFA Authenticated or DFA Apostilled PSA issued Marriage Certificate/ Contract or Report of Marriage (previous spouse) – Original and four (4) copies
- DFA Authenticated or DFA Apostilled PSA issued Advisory on Marriages – Original and four (4) copies
(make sure that the details in the PSA Advisory on Marriages are consistent or the same with the PSA Birth Certificate) – valid only with six (6) months from the date of issuance - DFA Authenticated or DFA Apostilled PSA issued Birth Certificate – Original and four (4) copies
- Death Certificate of the Deceased Spouse:
◦ If Filipino – DFA authenticated or DFA Apostilled PSA issued Death Certificate – Original and four (4) copies
◦ If Japanese – Koseki Tohon – Original and four (4) copies
◦ If foreign national, other than Japanese – Death Certificate issued by his/ her Embassy or Consulate. (must be translated in English) – Original and four (4) copies
Application by Mail
- Fill-out the Report of Marriage Form, print four (4) original copies, then sign on each form.
• All the details must be true and accurate.
• All the details must be type-written and in BLOCK letters.
• The forms must be printed in A4-sized paper.
• ROM form link: https://tokyo.philembassy.net/downloads/forms/rom-2018.pdf - Email the application form along with the required documents to civilreg@DELphilembassy.net for pre-evaluation.
- After pre-evaluation is complete, bring the ROM forms to a Japanese Notary Public for notarization
• Each form must have the signature of the registrants.
• Only one (1) notarial certificate is required for the same document.
• There is no need for the Notary Public to sign/stamp on the forms. - Send these forms to the Embassy, along with the other required documents as listed above, including a self-addressed 520 letterpack

Note: Registrants are NOT required to forward their original passports and/or other valid IDs to the Embassy. However, its clear and readable photocopies must be provided. - Once the physical documents have been verified to be complete and authentic, an officer will contact the registrants, either by email or through phone, to inform about the necessary consular fees which will be paid through Genkin Kakitome.
Reminders
- All photocopies should be in A4 sized paper.
- Application may be submitted in person or by mail – if by mail, the application form must be notarized by a Japanese notary public;
- The Embassy may require additional documents if deemed necessary.
- Applicable fees depend on each particular case.
- Processing period is 10 working days upon receipt of complete documents.
