Special Cases for Passport Application

Lost Passport, Assumed Identity and Mutilated Passport Cases are considered special cases and are treated as NEW APPLICATIONS.

FOR LOST PASSPORT APPLICATIONS:
Please prepare the following documents and requirements:

  • Confirmed Online Appointment (click here to schedule)
  • Special Cases Application form (click here to download)
  • Accomplished Application Form – Please print the system-generated application form from the online appointment system.
  • Affidavit of Loss (download the form here)
  • Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
    • Married Females using their spouse’s last name must also present the Original and submit a photocopy of either the:
      – PSA Authenticated Marriage Contract on Security Paper; or
      – Report of Marriage
  • Photocopy of data page of latest passport (if available)
  • Self-addressed ¥600 Letterpack
  • Police Report in English (Original and photocopy)
    (Translation of the Police Report can be applied for in the Embasy)
  • Two (2) Valid IDs with photocopy (click here for a list of valid IDs)
  • Payment of JPY ¥30,000 (includes Penalty Fee for lost passport)

Important Note:
–  If the PSA Birth Certificate or Marriage Certificate is unclear or unreadable, a copy from the Local Civil Registrar is required. 

Personal appearance and submission of all required documents are mandatory on your scheduled appointment date.

FOR ASSUMED IDENTITY CASES:
Applicants falling under assumed identity cases must email the Passport section at passport@philembassy.net with the subject line: “Special Cases: Assumed Identity” for  pre-processing of the following requirements:

  1. Appointment through email passport@philembassy.net or submission of their documents at the Embassy for initial assessment
  2. Special Cases Application form (click here to download)
  3. Accomplished Renewal Application Form (click here to download)
  4. Affidavit of Explanation (click here to download)
  5. Personal Appearance
  6. Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
    • Married Females (who are using their spouse’s last name) must also present Original and submit photocopy of PSA Authenticated Marriage Contract on Security Paper or Report of Marriage
    • Local Civil Registrar Copy is required if PSA Birth Certificate is not clear or cannot be read
  7. School Records such as Form 137, Transcript of Records
  8. Baptismal Certificate
  9. Two (2) Valid IDs with photocopy (click here for a list of valid IDs)
  10. Other documents to establish identity

FOR MUTILATED OR DAMAGED PASSPORTS:

Note: In cases where the passport presented for renewal is visibly damaged or mutilated, the following requirements shall apply instead of the regular renewal application requirements.

  1. Appointment through email passport@philembassy.net
  2. Accomplished Application Form – Please print the generated application form from the online appointment system.
  3. Affidavit of Mutilation (click here to download)
  4. Personal Appearance
  5. Photocopy of data page of latest passport (if available)
  6. Self-addressed ¥600 Letterpack
  7. Payment of JPY ¥30,000 (includes Penalty Fee)
