The Consular Officer reserves the right to require additional documents, if and when necessary, due to discrepancies. Submission of fake documents shall be dealt with in accordance with law.

    1. If changing from maiden to married name after marriage:
      1. PSA-issued Marriage Contract or original Report of Marriage issued less than 1 year before date of renewal by the Philippine Embassy in Tokyo.
    2. If changing name due to adoption in Japan:
      1. Annotated Birth Certificate issued by PSA with new name; AND
      2. DFA-authenticated copy of the Court Decision with Entry of Judgement from a Philippine Court recognizing the adoption in Japan
    3. If changing name to revert to maiden name after annulment/judicial recognition of divorce:
      1. PSA-issued Marriage Contract with Annotation to show annulment/divorce/court-ordered instruction, OR

      2. If annotated Marriage Contract from PSA is not yet available,

        • applicant must submit a DFA-authenticated Certified True Copy of the Court Order dissolving the marriage, AND

        • a DFA-authenticated Certificate of Finality from the court.

    4. If reverting to maiden name due to death of a foreigner spouse:
      1. PSA-issued Death Certificate of deceased Filipino spouse; or
      2. Death Certificate of Foreign Spouse authenticated by the Foreign Government concerned.

  1. Confirmed appointment. (There is a clearing period of 15 working days from the date of interview for assumed identity or passport discrepancy, and for verification of passport records before actual processing of the application.)

  4. Original copy of Philippine Statistics Authority (PSA) Birth Certificate on Security paper. Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate or Report of Birth are unreadable.
  5. Married females must also submit original copy of PSA-issued Marriage Contract on security paper or original Report of Marriage issued less than 1 year before date of renewal
  6. One of the following valid IDs with one (1) photocopy:

SUPPORTING DOCUMENTS

  1. In case of Late Registered Birth Certificate:
    • If Birth Certificate was registered at least ten (10) years ago, application will be treated as a regular application.
    • If Birth Certificate was registered less than ten (10) years ago, applicant must submit IDs that pre-date the late registration.
    • If applicant has no IDs that pre-date the late registered Birth Certificate, applicant must produce current IDs and National Bureau of Investigation (NBI) Clearance.
  2. In case applicant has no Birth Certificate or Report of Birth
    • If applicant is born AFTER 1950, applicant must first file for late registration with the Local Civil Registrar with jurisdiction over the place where applicant was born. Applicant will then submit the original copy of the PSA-issued late registered Birth Certificate with the supporting documents and IDs that pre-date the late registration.
    • If applicant is born ON OR BEFORE 1950, applicant must submit original copies of PSA-issued, DFA-authenticated Certificate of No Birth Record AND Affidavit of Two Disinterested Persons attesting to the identity of the applicant. Affidavit must be DFA-authenticated if executed in the Philippines.
  3. In case applicant has lacking data in Birth Certificate or Report of Birth
    • Applicant must submit proof of filing of supplemental report or correction from the Local Civil Registrar
  4. In case applicant has discrepancy in data in Birth Certificate/Report of Birth and other documents:
    • Discrepancy in first name:
      • Applicant must submit a copy of the petition and supporting documents for correction of first name filed with the Local Civil Registrar of consulate
    • Discrepancy in other data:
      • Applicant must submit annotated PSA-issued birth certificate with annotation reflecting the corrected entry.
  5. If applicant is a Dual Citizen:
    • Original PSA-issued Birth Certificate / Report of Birth
    • Original Identification Certificate issued by the Philippine Foreign Service Post (FSP) or the Bureau of Immigration (BI) and a photocopy
    • Government-issued IDs (from the Philippines or from country of second citizenship)
  6. If applicant is a Naturalized Filipino Citizen:
    • Identification Certificate of Naturalization from the Bureau of Immigration
  7. If applicant obtained Filipino citizenship by election:
    • Affidavit of Election of Filipino Citizenship
    • Identification Certificate of Election from the Bureau of Immigration
  8. If applicant has been granted citizenship by Act of Legislation:
    • DFA-authenticated Certified True Copy of law granting citizenship
    • Foreign Birth Certificate authenticated by the Philippine Foreign Service Post concerned.
