Requirements for Passport Renewal
- Personal appearance of applicant (for biometric data capture);
- Duly accomplished application form available at the Embassy, or downloadable from the official Embassy website;
- Passport for renewal;
- Photocopy of data page of passport being renewed, including amendment page if applicable;
- If applicant is a minor (below 18 years of age), photocopy of the data page of natural parent/legal guardian’s valid passport.
Note: Releasing of passports shall be by MAIL only. For this purpose, applicants need to bring one self-addressed, prepaid LetterPack 500 envelope for the return of their new passport and related documents.

Applicants are advised to bring at least a copy of their NSO birth certificate for verification of personal details when applying for renewal.
Additional Information:
- Holders of brown passports must present his/her NSO authentic birth certificate and at least one (1) identification document when applying for passport renewal.
- Seafarers must present the following for passport renewal, in addition to the requirements above:
- Certification from the corresponding shipping agency in Japan or from the ship master
- Philippine Seaman’s discharge book
- Landing permit issued by Japanese immigration
- Applicants with discrepancies in their passports need to undergo an evaluation procedure and at least two interviews prior to renewal. Additional documents may be required from the applicant on a case-to-case basis as determined by the consular officer. The appropriate form can be requested from the Passport window.
