Guidelines for Individual Seaman Visa

  1. Duly accomplished application form for Non-Immigrant Visa (FA Form No. 2);
  2. One passport size photograph (4.5 cm x 3.5 cm);
  3. Passport with a validity of at least six (6) months beyond the intended period of stay in the Philippines;
  4. Seaman’s book and photocopy of the pertinent pages;
  5. Letter of Recommendation from the General Manager, Operations Department of the Shipping Agents in Japan to be authenticated by the Philippine Embassy;
  6. Letter of Invitation from the Manning Agent in the Philippines

NOTE :
Prior to submission of documents, item no. 5 (Letter of Recommendation) must be notarized and authenticated first by the Japanese Ministry of Foreign Affairs (GAIMUSHO). Please refer to the Embassy’s authentication procedure (Ninsho Tetsuzuki).

Visa for approved application will be released after five (5) working days (excluding date of application and public holidays).
