Guidelines for Crewlist Visa
- Application Form for Crew List Visa duly accomplished in triplicate;
- Original Letter of Recommendation from the General Manager, Operations Department of the Shipping Agent in Japan [with two (2) photocopies] to be authenticated by the Philippine Embassy;
- Captain’s Specimen Signature [with two (2) photocopies] to be authenticated by the Philippine Embassy;
- Photocopies [three (3) sets] of Passport or Seaman’s Book of the Captain of the vessel;
- Photocopies of the passports or seaman’s books of the crew members [three (3) sets]
NOTE :
Prior to submission of documents, item no. 2 (Letter of Recommendation) and item no. 3 (Captain’s Specimen Signature) must be notarized and authenticated first by the Japanese Ministry of Foreign Affairs (GAIMUSHO). Please refer to the Embassy’s authentication procedure (Ninsho Tetsuzuki).
Visa for approved application will be released after five (5) working days (excluding date of application and public holidays).
