Requirements for Passport Renewal

Regular renewal of ePassports (dark maroon, with ePassport symbol):epassport

  1. Personal appearance of applicant.
  2. Fully- accomplished application form. (may be downloaded from Embassy website or fill-up online)
  3. Passport for renewal.
  4. Copy of passport photo/data page and amendment page, if applicable.
  5. One (1) self-addressed Japan Post ¥510 Letter Pack envelope.
  6. Passport fee of ¥7,800. (Note: Fees are payable in US Dollar or its equivalent in Japanese Yen as computed by our Cashier.)
  7. Additional requirements listed below if it applies to the passport applicant.

Note: Please make all photocopies on A4-size paper before arriving at the Embassy.

ADDITIONAL REQUIREMENTS FOR THE FOLLOWING:

Brown passports or passports issued before 01 May 1995:

  1. Copy of passport pages 1,2,3,4 and last page
  2. Original and copy of Philippine Statistical Authority (PSA)/ National Statistics Office (NSO) Birth Certificate (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate are unreadable.

Machine-Ready Readable Passports (MRRP) or green passports:mrrp

  1. Copy of passport photo/data page and last page
  2. Original and copy of PSA/NSO Birth Certificate (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate are unreadable.

Machine Readable Passports (MRP):mrp

  1. Copy of passport photo/data page and last page
  2. Original and copy of PSA/NSO Birth Certificate (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate are unreadable.

Minor applicants (below 18 years old):

  1. Valid passport and copy of passport photo/data page of parent/ legal guardian (Mother only if the child’s parents are not married. If mother cannot be present, authorization and valid ID should be presented by the father or legal guardian.)
  2. Original and copy of minor’s PSA/NSO Birth Certificate or Report of Birth (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate are unreadable.
  3. Parent or legal guardian’s personal appearance

Married women who wish to use their married surname:

Original and copy of PSA/NSO Marriage Certificate or Report of Marriage (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Marriage Certificate or Report of Marriage are unreadable.

Note: Once a married woman uses the surname of her husband, she cannot revert to her maiden name unless widowed, divorced with Philippine court’s judicial recognition, or marriage is annulled.

Formerly married women who wish to revert to their maiden name:

Original and copy of PSA/NSO Birth Certificate (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate are unreadable.

If widowed: Original and copy of PSA/NSO Death Certificate of the deceased Filipino spouse (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Death Certificate are unreadable; or Koseki Tohon for Japanese spouse.

If annulled:

  1. Original and copy of the DFA-Authenticated Philippine Court Order effecting the annulment
  2. Original and copy of annotated PSA/NSO Marriage Certificate or Report of Marriage (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Marriage Certificate/ Report of Marriage are unreadable.
  3. Original and copy of PSA/NSO Advisory on Marriages (issued less than 6 months before date of passport application).

If divorced:

  1. Original and copy of the DFA-Authenticated Philippine Court Recognition effecting the foreign divorce
  2. Original and copy of annotated PSA/NSO Marriage Certificate or Report of Marriage (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Marriage Certificate or Report of Marriage are unreadable.
  3. Original and copy of PSA/NSO Advisory on Marriages

Legally Adopted:

  1. Original and copy of PSA/NSO Birth Certificate or Report of Birth (issued less than 6 months before date of passport application) before adoption (if available)
  2. Original and copy of the amended PSA/NSO Birth Certificate or Report of Birth reflecting the new adoptive surname (issued less than 6 months before date of passport application)
  3. Original and copy of DFA-Authenticated Philippine Court Order on Adoption and Certificate of Finality

Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225 (The Citizenship Retention and Re-acquisition Act of 2003)

  1. Original and copy of PSA/NSO Birth Certificate or Report of Birth (issued less than 6 months before date of passport application). Local Civil Registrar (LCR) transcription is required when entries in the Birth Certificate or Report of Birth are unreadable.
  2. Original and copy of Oath of Allegiance
  3. Original and copy of Order of Approval
  4. Original and copy of Identification Certificate (if available)
