Requirements for Passport Renewal

  1. Personal appearance of applicant.
  2. Duly accomplished application form. (may be downloaded from Embassy website or fill-up online)
  3. Passport for renewal.
  4. Photocopy of the data page (the page with photo) and the last page of passport. Also photocopy of the amendment page, if applicable.
  5. Letter Pack 500 envelope.
  6. Passport fee. (Note: Fees are payable in US Dollar or its equivalent in Japanese Yen as computed by our Cashier.)

ADDITIONAL REQUIREMENTS:

For holders of brown passports or passports issued before 01 May 1995:

Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page.

NSO Birth Certificate. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.

For holders of Machine-Ready Readable Passports (MRRP) or green passports:

Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.

For holders of Machine Readable Passports (MRP):

Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.

NSO Birth Certificate to verify complete place of birth. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.

Old cancelled passport may be presented in the absence of an NSO Birth Certificate.

For holders of Electronic Passport (E-passport):

Bring original passport and photocopy of the data page and the last page of passport. Also photocopy of the amendment page, if applicable.

For married women who wish to use their married surname:

Report of Marriage or NSO Marriage Certificate. Transcribed Marriage Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Marriage Certificate are blurred or unreadable.

For formerly married women who wish to revert to their maiden name:

NSO Birth Certificate. Transcribed Birth Certificate from the Local Civil Registrar (LCR) is required when entries in the NSO Birth Certificate are blurred or unreadable.

If widowed: Original and photocopy of NSO Death Certificate of the deceased spouse (if the spouse is Filipino) or Koseki Tohon (if the spouse is Japanese).

If annulled: Original and photocopy of the Philippine Court Order effecting the annulment and annotated NSO Marriage Certificate or Report of Marriage.

If divorced: Original and photocopy of the Philippine Court Recognition effecting the foreign divorce and annotated Marriage Certificate or Report of Marriage.

For minor applicants (below 18 years old):

Valid passport of the parents or legal guardian and photocopy of the data page of said passport.

Photocopy of the data page of the minor child’s passport to be renewed.

NSO-certified Birth Certificate of the Child, or the original Report of Birth issued by the Philippine Embassy in Tokyo.

The minor applicant must be accompanied by the parent or legal guardian.

For the Legally Adopted:

Original and photocopy of the amended Birth Certificate reflecting the new adoptive surname.

Original and photocopy of the NSO Birth Certificate before adoption (if available).

Original and photocopy of the Philippine Court Order on Adoption and Certificate of Finality.

For applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225 (The Citizenship Retention and Re-acquisition Act of 2003):

NSO Birth Certificate or Report of Birth.
Oath of Allegiance.
Order of Approval.
Identification Certificate (if available).

NOTE: 

  1. All NSO documents should be in security paper.
  2. All photocopies should be in A4 size.
  3. The Consular Officer reserves the right to require additional proof or documents, pursuant to R.A. 8239 (The Philippine Passport Act of 1996) for the purpose of verifying the identity, citizenship and personal circumstances of the applicant.
