Passport Application for Adopted Filipinos

The adoption of Filipino children must be done in accordance with the provisions of R.A. 8043, also known as the ‘Inter-Country Adoption Act of 1995’.

All adoption cases should be endorsed to the Inter-Country Adoption Board (ICAB), through the International Social Service of Japan (ISSJ) located at Ochanomizu K & K Building 3F, 1-10-2 Yushima, Bunkyo-ku, Tokyo 113-0034 (Tel.No. 03-5840-5711 Fax No. 03-3868-0415)

Qualified Filipinos who have gone through the process of legal adoption may be issued passports reflecting the new adoptive surname after submitting the following:

  1. Amended NSO Birth Certificate (showing the new adoptive surname) in security paper authenticated by DFA Manila.
  2. Old NSO Birth Certificate (using the original surname) in security paper.
  3. Certificate of Registration of Adoption (issued by Manila Local Civil Registrar).
  4. Photocopy of Authenticated Philippine Court Decree of Adoption.
  5. Old passport.

Pursuant to Sec.48, Rule 39 of the Rules of Court and the Supreme Court ruling in G.R. 154380 dated 5 Oct 2005, orders and judgments rendered by foreign courts must be passed upon judicially by Philippine courts to prove its validity. These foreign judgments, which include adoption (except those that passed through ICAB) must be judicially confirmed by filing a civil action at the RTC courts in the Philippines.
