(MINOR APPLICANTS)

IMPORTANT NOTICE:

Please make sure that you have read this page thoroughly and that you have all the requirements before going to the Consular Section of the Embassy for your renewal.

The Consular Officer reserves the right to require additional documents, if and when necessary, due to discrepancies. Submission of fake documents shall be dealt with in accordance with law.

NEW APPLICATION FOR MINOR APPLICANTS

RENEWAL OF PASSPORTS OF MINOR APPLICANTS

MINOR APPLICANT SHOULD BE ACCOMPANIED BY A PARENT.
    1. Personal appearance of the minor
    2. Personal appearance of either parent (for a legitimate child).
    3. Accomplished Application Form (REGULAR PASSPORT APPLICATION FORM (MINOR))
    4. PSA-issued birth certificate or original Report of Birth (if reported in the Philippine Embassy in Tokyo and issued less than 6 months before date of renewal)
    5. Valid Passport of accompanying parent + 1 photocopy of the passport data page
    6. School ID (if available)
    7. Self-addressed Letter Pack 510 (if passport will be mailed to you)
    8. Passport Fee (See Schedule of Fees)
RENEWAL OF PASSPORTS OF MINOR APPLICANTS In case legitimate minor child is not accompanied by parents during the application process
  1. Personal appearance of the minor
  2. Personal appearance of authorized adult companion.
  3. Special Power of Attorney executed by the parents designating the minor’s companion to assist the minor in the application process.
  4. Accomplished Application Form (REGULAR PASSPORT APPLICATION FORM (MINOR))
  5. Valid passport or valid government-issued ID of Authorized Adult Companion, plus 1 photocopy of data page.
  6. PSA-issued birth certificate or original copy of the Report of Birth (if reported in the Philippine Embassy in Tokyo and issued less than 6 months before date of renewal)
  7. Valid Passport of accompanying parent
  8. School ID (if available)
  9. Self-addressed Letter Pack 510 (if passport will be mailed to you)
  10. Passport Fee (See Schedule of Fees)
NEW APPLICATION FOR MINOR APPLICANTS

RENEWAL OF PASSPORTS OF MINOR APPLICANTS

In case applicant is an illegitimate child in custody of the Mother
  1. Personal Appearance of minor applicant
  2. Personal appearance of the mother or authorized adult companion.
    1. If applicant is not accompanied by mother during the application process, applicant must submit a Special Power of Attorney executed by the mother designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad.
  3. Accomplished Application Form (REGULAR PASSPORT APPLICATION FORM (MINOR))
  4. PSA-issued Birth Certificate, or original Report of Birth  (if reported in the Philippine Embassy in Tokyo and issued less than 6 months before date of renewal)
  5. Passport or Valid Government-issued ID of mother or authorized Adult Companion

  4. Valid Passport or valid government ID of adult guardian
  5. DSWD Clearance
  6. Affidavit of Guardianship

NEW APPLICATION FOR MINOR APPLICANTS

RENEWAL OF PASSPORTS OF MINOR APPLICANTS

If minor applicant is undergoing or has undergone the process of adoption in the Philippines
  1. Personal Appearance of minor applicant
  2. Personal appearance of adoptive parents
    1. If applicant is not accompanied by adoptive parents during the application process, applicant must submit a Special Power of Attorney executed by the adoptive parents designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad.
  3. Accomplished Application Form (REGULAR PASSPORT APPLICATION FORM (MINOR))
  4. PSA-issued Birth Certificate
  5. Valid passport or valid government issued ID of adoptive parents
  6. DFA-authenticated Court Decree of Adoption
  7. Self-addressed Letter Pack 510 (if passport will be mailed)

If minor applicant is undergoing or has undergone the process of foreign adoption/Inter Country Adoption Board (ICAB) adoption process
  1. Personal Appearance of minor applicant and ICAB representative
  2. Accomplished Application Form (REGULAR PASSPORT APPLICATION FORM (MINOR))
  3. PSA-issued Birth Certificate/PSA Certificate of Foundling
  4. Endorsement from ICAB
  5. Certificate of Child Available for Adoption/Deed of Voluntary Commitment
  6. Placement Authority issued by ICAB
  7. Certificate for Issuance of Passport issued by ICAB
  8. Clearance for Inter-Country Adoption
  9. Child Study Report
  10. DSWD clearance
  11. Self-addressed Letter Pack 510 (if passport will be mailed)

If Minor applicant is a foundling NOT for adoption
