Requirements for Single Filipino

  1. NSO Birth Certificate authenticated by DFA Manila – Red Ribbon (Original & 1 Photocopy)
  2. Valid Passport: Original & one (1) photocopy of the data page.
    NOTE: If Expired Passport/Lost Passport/Mutilated Passport/Passport under an Assumed Name (AKA/Alias)/Passport with Discrepancy (First Name/Middle Name/Last Name/Date of Birth/Place of Birth), applicant has to apply for a new passport prior to acceptance of application for CNO.
  3. NSO Certificate of Non-Availability of Record of Marriage [CENOMAR] (Purpose: For Marriage), authenticated by DFA-Manila-Red Ribbon (Original & 1 Photocopy) NOTE: The validity of CENOMAR is six (6) months from the date of issue of the NSO.
  4. If 18-20 years old, Affidavit of Parental Consent or if 21-25 years old, Affidavit of Parental Advice. It should indicate name of fiancé/fiancée (Original & 1 Photocopy). If the parent/s are in the Philippines, these Affidavits have to pass through:
    1. Notary Public in the Philippines
    2. Regional Trial Court
    3. DFA Manila, Authentication Division
      If parent/s are in Japan, affidavits of consent and parental advice may be filed personally by the parent/s at the Embassy. If one or both parents are deceased, please provide Death Certificate issued by NSO.
  5. Two (2) Passport Size Pictures

Requirements for Japanese National

  1. Family Registry (Koseki Tohon) (Original & 1 Photocopy) Validity is 3 months from date of issue.
    1. If the applicant is a divorcee, the record of marriage and divorce with previous spouse should be indicated in the koseki tohon.
    2. If the applicant is a widow/widower, the record of death of the previous spouse should be indicated in the koseki tohon.
    3. A Koseki Shohon is not acceptable.
  2. Original and one (1) Photocopy of Passport or Driver’s License or National Health Insurance
  3. Two (2) Passport-Size Pictures

Requirements for Foreign National other than Japanese

  1. A Certificate of No Objection (CNO) or its equivalent from the concerned Embassy or Consulate (Original & 1 Photocopy). The foreign national has to obtain the CNO or its equivalent from his/her respective Embassy/Consulate. The CNO or its equivalent must be in English.
    For US military personnel based in Japan, Authorization to Marry from the commanding officer is required.
  2. Passport or Driver’s License or Military ID (Original & 1 Photocopy)
  3. Two (2) Passport-Size Pictures
