Requirements for Divorced Filipino (without Phil. Court Recognition)

  1. Advisory on Marriages issued by NSO (Original & 1 Photocopy)
  2. NSO Birth Certificate authenticated by DFA-Manila-Red Ribbon (Original & 1 Photocopy)
  3. Valid Passport: Original & One (1) photocopy of the data page.
    NOTE: If Expired Passport/Lost Passport/Mutilated Passport/Passport under an Assumed Name (AKA/Alias)/Passport with Discrepancy (First Name/Middle Name/Last Name/Date of Birth/Place of Birth), applicant has to apply for a new passport prior to acceptance of application for CNO.
  4. Alien Certificate of Registration (ACR) & (1) photocopy
  5. Original and one (1) photocopy of the following to be obtained from the City Hall:
    1. Family Registry (Koseki Tohon) reflecting the couples’ place and date of divorce if former spouse is a Japanese. Please note that Koseki Shohon (part of Koseki Tohon) and Juri Shomeisho (Certificate of Acceptance) are not acceptable.
    2. Certificate of Acceptance of Divorce (Juri Shomeisho) showing the couples’ place and date of divorce if former spouse is a foreign national other than Japanese.
  6. Original and one (1) photocopy of Marriage Contract with previous spouse
    1. Marriage Contract issued by NSO if married in the Philippines
    2. Report of Marriage issued by Philippine Embassy / Consulate General if married in Japan or other country
  7. Certificate of Philippine Nationality/Identity (CPNI) and Affidavit of Undertaking is required if the surname in the passport is still under the surname of the previous spouse. ( Forms can be secured at the Embassy )
  8. Three (3) Passport Size Pictures

Requirements for Japanese National

  1. Family Registry (Koseki Tohon) (Original & 1 Photocopy) Validity is 3 months from date of issue.
    1. If the applicant is a divorcee, the record of marriage and divorce with previous spouse should be indicated in the koseki tohon.
    2. If the applicant is a widow/widower, the record of death of the previous spouse should be indicated in the koseki tohon.
    3. A Koseki Shohon is not acceptable.
  2. Original and one (1) Photocopy of Passport or Driver’s License or National Health Insurance(with a Certified Resident Card (Juminhyo))
  3. Two (2) Passport-Size Pictures
