Lost Passport / Mutilated Passport / Passport Under an Assumed Name (AKA/Alias) / Passport with Discrepancy (First Name/Middle Name/Last Name/Date of Birth/ Place of Birth)

NOTE: Personal appearance is a must. ONE (1) MONTH clearing period for investigation and approval from DFA, Manila is required. A New Passport may be issued to an applicant only after official endorsement/approval from DFA is received and after applicant has complied with all documentary requirements.

  1. Duly accomplished passport application form available at the Consular Information Counter, or downloadable from the official Embassy website at http://tokyo.philembassy.net
  2. Original & One copy of Birth Certificate in security paper issued by the NSO. It should be authenticated by the Department of Foreign Affairs (DFA) – Authentication Division.
  3. In case of Late Registration of Birth, Certified True Copy of the Certification of No Birth Record issued by the Local Civil Registrar when applicant filed for late registration of birth.
  4. Original & One (1) copy of Baptismal Certificate with Dry Seal. It should be authenticated by Department of Foreign Affairs (DFA) – Authentication Division.
  5. Original & One (1) copy of Form 137 (Elementary or High School)/Yearbook or Transcript of Records w/ Dry Seal. It should be authenticated by Department of Foreign Affairs (DFA) – Authentication Division
  6. Original & One (1) copy of Marriage Contract in security paper issued by the NSO (If married in the Philippines). It should be authenticated by Department of Foreign Affairs (DFA) – Authentication Division
  7. Original & One (1) copy of Report of Marriage (If married in Japan or in another country)
  8. Original & One (1) copy of Report of Birth (Issued by the Philippine Embassy)
  9. Original & One (1) copy of Travel Document/Certificate of Philippine Nationality and Identity (CPNI)
  10. Original & One (1) copy of Police Report from the concerned Police Station (If lost passport has a valid visa)

NOTE: Submission of above item nos. 1 – 4 is required from all applicants. Submission of item nos. 5 – 10 is required if applicable to the applicant. If the information appearing in your birth certificate is inconsistent with other documents submitted (Ex. first name, middle name, last name, date of birth, place of birth), late registration and/or with an assumed name, the issuance of a new passport may be temporarily held in abeyance pending the submission of the ORIGINAL AND A PHOTOCOPY OF AT LEAST TWO (2) OLD IDENTIFICATIONS/DOCUMENTS, as follows:

  • Original & One (1) copy of School Yearbook / School ID
  • Original & One (1) copy of COMELEC Voter’s Affidavit and/or Voter’s Identification Card or Certified True Copy of Voter’s Registration Record
  • Original & One (1) copy of National Bureau of Investigation (NBI) Clearance (Old)
  • Original & One (1) copy of Land Titles
  • Original & One (1) copy of Passbook/Bankbook
  • Original & One (1) copy of Driver’s License
  • Original & One (1) copy of Proof of Service with the Philippine Government
  • Original & One (1) copy of GSIS/SSS records (SSS Information Sheet)
  • Original & One (1) copy of BIR’s TIN card
  • Original & One (1) copy of Naturalization Certificate
  • Original & One (1) copy of Death Certificate of spouse/parents/children
  • Original & One (1) copy of Seaman’s Book
  • Original & One (1) copy of Professional Regulatory Commission (PRC) Identification Card
  • Artist Record Book (ARB) issued by the Philippine Overseas Employment Administration (POEA)
  • Original & One (1) copy of Certificate of Identity and Tribal Membership issued by the Office of the Muslim Affairs
  • Original & One (1) copy of Counseling Certificate issued by the Commission on Filipino Overseas (CFO)
  • Original & One (1) copy of any other official and genuine document that may contain information about the applicant.
