Amendment of Passport
A. From Single Name to Married Name
- Duly accomplished amendment form
- Passport (Original and 1 photocopy each of 1st page, visa page & last page)
- If married in the Philippines:
Marriage Contract in security paper issued by the NSO authenticated by Authentication Division, DFA Manila (Original and 1 Photocopy)If married in Japan or another country:
Report of Marriage from the Philippine Embassy in Tokyo/Philippine Consulate General in Osaka or from the Embassy or Consulate in the country where the marriage took place (Original and 1 Photocopy)
Amendment Fee: US$20
Note: Fees are payable in US Dollar or it’s equivalent in Japanese Yen. Please consult the Embassy for the current applicable rate.
B. From Married Name to Single Name
- Duly accomplished amendment form
- Passport (Original and 1 photocopy each of 1st page, visa page & last page)
- For Filipinos with annulled marriage:
- Amended Marriage Contract in security paper issued by the NSO authenticated by Authentication Division, DFA Manila (Original and Photocopy)
- Certificate of Finality of Decision and Court Order/Decision authenticated by Authentication Division, DFA Manila (Original and Photocopy)
For Widowed Filipinos:
- If spouse is a Filipino: Death Certificate of Previous Spouse in security paper issued by the National Statistics Office (NSO) authenticated by Authentication Division, DFA Manila (Original and 1 Photocopy).
- If spouse is a Japanese: Family Registry (Koseki Tohon) reflecting the Death of the husband/wife (Original and 1 Photocopy)
- If spouse is a Foreign National other than Japanese:
Death Certificate (including English translation, if applicable) from the concerned Embassy (Original and 1 Photocopy)
- If married in the Philippines:
Marriage Contract with previous spouse in security paper issued by the National Statistics Office (NSO), authenticated by Authentication Division, DFA Manila (Original and Photocopy)If married in Japan or another country:
Report of Marriage issued by the Philippine Embassy in Tokyo/ Philippine Consulate General in Osaka or from the Embassy or Consulate in the country where the marriage took place. (Original and 1 Photocopy) - Birth Certificate in security paper issued by the National Statistics Office (NSO) (Original and 1 Photocopy)
NOTE: Amendment of passport can only be applied once to a current and valid passport. Further amendment will require application for a new passport.
Amendment Fee: US$20
Verification of Koseki Tohon: US$25
