INTER-COUNTRY ADOPTION SHOULD BE IN ACCORDANCE WITH REPUBLIC ACT NO. 8043, OTHERWISE KNOWN AS “THE INTER-COUNTRY ADOPTION ACT 1995”. ALL ADOPTION CASES SHOULD BE ENDORSED TO THE INTER-COUNTRY ADOPTION BOARD (ICAB) FOR APPROVAL THROUGH THE INTERNATIONAL SOCIAL SERVICE OF JAPAN (ISSJ).PURSUANT TO R.A. NO. 8043, A FILIPINO CHILD WHO CAN BE LEGALLY ADOPTED SHOULD BE BELOW FIFTEEN (15) YEARS OF AGE.

After the decree of adoption is issued, the Filipino parent has to register the adoption in the Philippines prior to issuance of a new passport to the adopted child reflecting the adoptive surname.
PROCEDURE FOR REGISTRATION OF ADOPTION
- Obtain the Court Decree of Adoption (Original Transcript) from the concerned Japanese Family Court;
- For information on documentary requirements and procedures, please contact ISSJ or ICAB after obtaining the court decree of adoption;
- Bring the Court Decree of Adoption to the Ministry of Foreign Affairs (Gaimusho) and apply for authentication;

- Bring the Court Decree of Adoption with English translation, and a photocopy of each document to Visa Section (Window 1), Philippine Embassy, for authentication;
Authentication Fee : US$25
Verification Fee : US$25 - Submit the authenticated Court Decree of Adoption to the Office of the Local Civil Registrar of Manila for registration of adoption; (The Local Civil Registrar of Manila will issue a certificate of registration and will forward the same to the Local Civil Registrar of place of birth of child which, in turn, will issue an amended birth certificate bearing the adoptive surname of the child. The Local Civil Registrar of place of birth will forward a copy of the amended birth certificate to the National Statistics Office (NSO) for registration.)
- Secure a copy of the amended birth certificate bearing the adoptive surname of the child from the National Statistics Office (NSO) and have it authenticated by the Authentication Division, Department of Foreign Affairs, Manila.
REQUIREMENTS FOR ISSUANCE OF A NEW PASSPORT
- Obtain the Court Decree of Adoption (Original Transcript) from the concerned Japanese Family Court;
- Birth Certificate (old surname) in security paper issued by the National Statistics Office (NSO) (Original & photocopy);
- Certificate of Registration of Adoption issued by the Local Civil Registrar of Manila (Original & photocopy) & photocopy of the Authenticated Court Decree of Adoption;
- Old Passport (Original & photocopies of 1st page, page with stamp of visa & last page);
- Three (3) passport-sized pictures (as specified in the passport application form);
- If applicant is a minor, submit photocopies of parents’ passports.
New Passport Fee: USD$50
