Certificate of Philippine Nationality / Identity (CPNI)

- Original and One (1) copy of applicant’s Birth Certificate in security paper issued by the National Statistics Office (NSO) authenticated by Authentication Division, DFA Manila

Certificate of Philippine Citizenship

- If born in the Philippines: Birth Certificate in security paper issued by the National Statistics Office (NSO) authenticated by Authentication Division, DFA Manila (Original and One (1) copy)
- If born in Japan or other countries: Report of Birth issued by the Philippine Embassy in Tokyo/Philippine Consulate General in Osaka or from the Embassy or Philippine Consulate in the country where the applicant was born.

Authentication of LTO Certification (Philippine Driver’s License)
- Duly accomplished Certification of Authenticity Form ? Available at the Consular Information Counter
- Original and photocopy of LTO Certification certifying the validity of a Philippine Driver’s License authenticated by Authentication Division, DFA Manila
- Photocopy of applicant’s Philippine Driver’s License
- Original and Photocopy of the following pages of applicant’s Passport:

NBI CLEARANCE
Applicant has to personally appear at the Embassy.
- Duly accomplished NBI Clearance Form with security barcode obtained by the applicant from the NBI in the Philippines. Applicants should request their authorized representative in the Philippines to send the NBI form to them in Japan.
- Original and Photocopy of the following pages of applicant’s Passport:
- First page (Data page) – 1 copy
- Visa page – 1 copy
- Last Page – 1 copy
- One (1) passport size picture
- Fingerprinting will be done after payment. Embassy personnel will assist in getting the fingerprints of the applicant.
- After the fingerprint impressions are taken at the Embassy, it is the responsibility of the applicant to transmit the application form with his/her prints to the NBI through an authorized representative. The application form must be submitted to NBI Manila for the issuance of the NBI Clearance Certificate. The Embassy merely assists in the taking of the fingerprint impressions.
